GO Active Get Healthy is funded by the Diabetes Transformation Fund. It is a partnership programme involving a range of stakeholders including the Oxfordshire Clinical Commissioning Group, Active Oxfordshire, and the Local Authorities; Cherwell District Council, Oxford City Council, South and Vale of White Horse Council and West Oxfordshire District Council.

The programme embeds the NHS Let's Get Moving Pathway into the primary care referral procedure for patients with diabetes to access behaviour change support initiatives county-wide.

Primary Care Team

The patient can be referred via the EMIS Exercise on Referral form on the right hand side or the patient can self sign up here.

The pathway includes telephone support, up to one year re-evaluation of goals and the latest resources from Moving Medicine.

Moving Medicine Ambassador

Leila joined Active Oxfordshire in August 2017 as part of the GO Active Get Healthy Team, a partnership funded programme with the Oxfordshire Clinical Commissioning Group and local District Councils. Leila co leads on disability and long term health conditions with a focus on developing behaviour change models to embed effective countywide, patient centred, physical activity pathways. Leila has previously coordinated a Sport England Community Activation Fund for Active South Leamington and worked for a local District Council for over 7 years.
